Case Management
An occupational therapist can assist in determining client needs and make recommendations for equipment, environmental modifications, and services to ensure maximum function, independence, and safety.
We will work with vendors and local providers to ensure your needs are met in a timely matter, and can follow up as needed on your behalf. This may include reports to parties involved, such as insurers.
Case management services can be provided on a short or long term basis. We can also assist with the transition between care environments, such hospital to home, or from home to assisted living or long-term care.
